I just want to ask about how to properly normalize this given table that I have. Here is my given table that I'm working on:

(third row with two blanks are still the data of Allen which has the subscriber number of 110)
(same goes with 5th and 6th row two blanks, data is still from its preceding term Smith which has the subscriber number 202)
(they were just left blank to identify them as the values from their preceding terms)
I am more likely very new and unfamiliar about the concepts of Normalization since I only recently learned about this for a bit in our class so please excuse me if I had made funny mistakes during my process of normalizing them in which I will present via pictures(since its more clear to see than having to input them one by one):
