Contact Us form html/php not working

Question

I found this code on the internet a while ago, and use it on my computer, but it does not actually work, it just displays what's written on the php file here's the code HTML:

what seem to be the problem here, i really cant make it work, btw the email address there is the test address directly from where i got the code, i already changed it to my email address but it's still not working for me
